Viz B — Screener: acting on hearing-screener patients before the Dx is booked
Everything that happens between "on the ENT schedule" and "diagnostic booked": who was invited, who answered, who scored positive and is still not booked, who needs a paper form at check-in, and who has gone dark. Same working pattern as Viz A: funnel → list → patient → act.
Rules on this screen: eligibility 50+ with no screener or test in 12 months (PFP deck Phase 1); SMS #1 at T−N, SMS #2 after 2–3 days of silence, paper at check-in if still unanswered (Mgt B, W3 line); Blue Form scoring per Ryan's ratified math (Q1=1, Q2=2, Q3=2, Q4=2, Q5 No=2 / N/A=1 / Yes=0, +1 breadth bonus at 3+ of Q1–Q4; Observer 0–2, Explorer 3–5, Driver 6–10; Q5=No forces Driver; the band never sets the color); positive = 2+ YES; a positive with no Dx booked is due for a call within one business day (PFP deck 1B), and two attempts on two channels without a reply is "gone dark". No form on file is NOT SCREENED, never "Observer, score 0".
